Service summary

We accept referrals from care managers where an individual is in either receipt of a completed Outcome-Based Support Plan (OBSP) with funding agreed or where a short term intervention is agreed to allow work to be completed / concluded re OBSP an agreed individual budget .

Referrals can be accepted from family members / self referrals as long as there are agreed OBSP with funding attached via Individual budget or Direct Payment .
