Supported Living services
Supported Living is designed to provide a more flexible care plan to support service users to remain in their own homes and prevent admission to hospital or long term residential care.
We provide home care services to support you based on your needs. The home care support you receive will be based on an assessment and is designed to:
- help you to achieve, regain or maintain a level of independence;
- improve your quality of life;
- provide access to enhanced telecare and
- support your family and friends in providing you with the care that you need.
Over time your needs may change, and we will adapt the service to reflect this. Your needs may change in a way that a traditional model of care is no longer meeting your needs effectively. Supported Living is a more flexible service that can be suitable to meet your needs if living at home with mainstream home care is no longer keeping you safe and allowing you or your family to cope with being at home. Supported Living can be an alternative to residential or long-term care depending on your needs.
The service operates 7 days per week between the hours of 7.30am and 10pm.
