Reablement

Reablement is a new service that provides tailored support to people in their own home for up to six weeks after discharge from Hospital.It builds confidence by helping people to regain the skills to dothings they can and want to do for themselves at home.

The Reablement service is a partnership between Social Work Services, Cordia Home Care and NHS Greater Glasgow and Clyde. The service is available to residents in all areas of Glasgow.

How it works

Reablement is based on you,your abilities and what you want to achieve. This involves:

  • Assessment

    Reablement starts with an assessment of what you can do, and what you would like to do, for yourself. The assessment is carried out by an Occupational Therapist.
  • Care Plan

    The Occupational Therapist agrees a plan for your Reablement with you and your home carer, based on your assessment. The amount of care and support you receive during and after the six week Reablement period will depend on your needs.
  • Weekly review

    The Reablement team will review your progress each week and update your plan in a care diary.The care diary is kept in your home, so that you, your family and carers can keep track of how your Reablement is progressing.

  • Final review

    A final review is carried out at the end of your Reablement period.After this review, you and your Reablement Team will agree your ongoing support needs and the Team will organise for them to be met.
  • Charges

    In line with current home care charging, underFree Personal Care, there is no charge for the Reablement service for anyone aged 65 and over. There may, however, be charges for other non-personal care services, for example, shopping.
